Poster 168: Notching of the Greater Tuberosity Revisited: A Radiographic Indicator of Degenerative Rotator Cuff Tears

چکیده

Objectives: The greater tuberosity notch (GTN) is an uncommonly reported radiographic finding which has been associated with articular sided partial rotator cuff tears (RCTs) in throwing athletes. This yet to be evaluated patients degenerative tears. purpose of this study assess the GTN measurement along five other previously described indicators for RTCs. Methods: After obtaining IRB approval, we prospectively collected and compared 80 standard shoulder radiograph series additional “notch view”, a lateral oblique showing profile. Thirty-four had confirmed via arthroscopy 46 control did not have suspected pathology. Two musculoskeletal radiologists independently reviewed radiographs size RCTs including: cyst (GTC), acromiohumeral interval (AHI), critical angle (CSA), inferior acromial sclerosis (IAS), spurring (LAS). A receiver operator characteristic (ROC) curve was performed compare individual performance each six markers. Nearest neighbor analysis based on Euclidean distances followed by clustering pattern variables occurring together setting RCT. Results: total 320 were 1920 measurements made. IAS, LAS, GTCs, decreased AHI large GTNs (P < 0.01), but CSA or smaller sizes. Overall, predicted RCT moderate accuracy (AUC=0.676). Cluster suggested strongest correlation most closely related 0.003) distances. Conclusions: presence medium-sized alone moderately sensitive detecting Additionally, at least one indicator dramatically increases likelihood during surgery. these markers combination may represent continuum increasing severity

MR imaging for traumatic tears of the rotator cuff: high prevalence of greater tuberosity fractures and subscapularis tendon tears.

OBJECTIVE The purpose of this study was to determine whether occult bony injuries or other characteristic MR abnormalities are frequent in patients suspected of having traumatic tears of the rotator cuff. SUBJECTS AND METHODS MR arthrography of the shoulder was performed in 24 consecutive patients with suspected traumatic tears of the rotator cuff. MR findings were analyzed with regard to abn...

Arthroscopic grafting of greater tuberosity cyst and rotator cuff repair.

Cysts of the greater tuberosity can be a normal finding independent of age and concurrent rotator cuff tear. The presence of a large greater tuberosity cyst can present a challenge at the time of rotator cuff repair. We present a 1-step arthroscopic technique to address these defects at the time of rotator cuff repair using a synthetic graft (OsteoBiologics, San Antonio, TX) originally designed...

Relationship of individual scapular anatomy and degenerative rotator cuff tears.

BACKGROUND The etiology of rotator cuff disease is age related, as documented by prevalence data. Despite conflicting results, growing evidence suggests that distinct scapular morphologies may accelerate the underlying degenerative process. The purpose of the present study was to evaluate the predictive power of 5 commonly used radiologic parameters of scapular morphology to discriminate betwee...

Bone density of the greater tuberosity is decreased in rotator cuff disease with and without full-thickness tears.

BACKGROUND Despite the high prevalence of rotator cuff disease in the aging adult population, the basic mechanisms initiating the disease are not known. It is known that changes occur at both the bone and tendon after rotator cuff tears. However, no study has focused on early or "pretear" rotator cuff disease states. The purpose of this study was to compare the bone mineral density of the great...

In vivo microstructural analysis of the humeral greater tuberosity in patients with rotator cuff tears using multidetector row computed tomography

BACKGROUND In arthroscopic surgery, the suture anchor technique has become popular for rotator cuff repair. Preoperative evaluation of the bone microstructure is of utmost importance because, especially in elderly patients, osteoporotic changes may cause anchor pullout, which results in failure of rotator cuff repair. Many groups have reported humeral microstructural analysis; however, most stu...
